Request to add hard stops to the H&P for required sections
The group felt that the hard stops would only be directed to a small percentage of users while simultaneously infuriating a larger percentage of our physician population. The group rejected the request, but would be willing to revisit the issue if a requestor could present data describing the magnitude of the problem.
The next CareNet+ iteration
Pranjal, Tahir, Bob and Chris volunteered to pilot the next version of CareNet+.
Anupam has asked the CareNet+ team to see if they can deliver notes to a user's CareConnection Office inbox rather than having to search for relevant documents within the patient's chart.
Reconcile and Plan functionality
The group would like to retract the Reconcile and Plan functionality as it is confusing to nurses and it exposes nurses to the same drug-drug and drug-allergy interactions that the physician has already addressed.
[] Anupam to ask the nurse liaison team to submit a change request to withdraw this functionality.
Key events documentation
The Lutheran team described the key events functionality in PowerNote. The key events could simplify building a discharge summary when the patient has seen by multiple providers during their hospital stay.
[] Chris to distribute a discharge summary built from key events.
Going paperless
Some sites are still having trouble with with Dragon profiles getting corrupted. The IT folks have assured Anupam that the problem has been resolved.
[] Site medical directors to contact Anupam and their local field IT teams if they can identify a user with ongoing issues with Dragon.
CPOE
The group asked that Anupam reach out to Nursing to make sure the nursing staff is knowledgeable about how to use the PowerPlans once the physician signs the initial order.
Non-invasive blood pressure mean arterial pressure
The group rejected the request to set reference ranges for mean arterial pressure since there are already flags when a patient's systolic or diastolic blood pressure is out of range.
ICD-10
Lee Sacks will be putting out a communication to all physicians in July about what Advocate is doing to support physician documentation efforts in ICD-10. The ICD-10 link within CareConnection takes the user to a website with CDI/coder-approved suggestions for the physician to consider before entering the assessment and plan in an H&P or when new diagnostic information is available.
Miscellaneous
Anupam has asked the Advance Directives Council about changing the requirement for a physical signature for LET orders.
Issues to track from prior meetings
[] If the signature + specialty request (with changing the resident's specialty with each rotation) and SmartTemplate work are performed by separate teams, then we will proceed down both paths simultaneously. Otherwise, we will work on the SmartTemplate request first before adding tropoinin in the daily labs SmartTemplate.
[] Anupam to update the website with tips & tricks on how to better use the documentation functionality within PC Touch
[] Anupam to ask Cerner for the estimated dates when H&P, operative notes and discharge summaries will be available in PC Touch (again)
[] Anupam to investigate options to pull in patient diagnoses on demand in PC Touch
The next meeting is scheduled for July 9th at 0730 hours.
